Overview

Cortico Patient Intake Forms help clinics collect essential information before appointments, reducing administrative workload and improving documentation accuracy. Forms are sent automatically when a patient books online, or manually sent by clinic staff for upcoming visits.

Patients complete forms from their mobile device or computer, and their responses sync back into the Accuro chart. Clinics can customize form content to match their workflows, specialties, and appointment types, ensuring the right information is captured every time.

This capability enhances efficiency by removing paper forms, improving data quality, and reducing bottlenecks at the front desk.

Note: This capability is part of the Cortico’s Premium Tool Suite, which includes online booking, reminders, intake forms, messaging, and video visits.

How it works:

The patient books an appointment (online or via clinic).
Cortico automatically sends the appropriate intake form, or staff can send it manually.
Patient completes the form securely online.
Responses are transmitted to the clinic and written back into Accuro.
Providers review form responses before or during the appointment.

- Demographics update (if applicable)

Data residency

Cortico data for Canadian clinics is hosted in secure Canadian data centers and remains within Canada.

Data usage

Form data is used solely to collect clinical and administrative information for the patient’s appointment. Data is not shared externally except as required to deliver the service. Retention of data in the EMR depends on EMR policies. Cortico generally only stores PHI in the EMR, but when it must, it retains data indefinitely to preserve the clinic’s records, until an event triggers redaction (clinic offboarding, patient request, etc.)
