Mika AI’s digital patient intake forms provide clinics with an efficient, patient-friendly way to collect information before appointments. Forms use an interactive & dynamic chatbot interface that adapts to each user’s responses and supports a wide range of clinical data types, including validated mental health screeners. Once completed, Mika AI generates a structured summary note for review and fills out patient information fields that can be pushed into Accuro to update patient contact information, custom fields, and much more.
How it works:
- Clinic sends a secure SMS/email link or attaches a form to a reminder.
- Patient completes the interactive questionnaire.
- Auto-scoring applies (e.g., PHQ-9, GAD-7).
- A structured summary is generated for clinician review.
- Summary can be added to the patient chart in Accuro.
