MyGut by Crohn’s and Colitis Canada is a digital health platform designed to support individuals living with Crohn’s disease or ulcerative colitis. It enables patients to track symptoms, lifestyle habits, flare activity, quality-of-life indicators, and health patterns over time.
For participating clinics, the MyGut Provider Portal connects patient-reported data with select Accuro EMR data (read-only), providing a longitudinal view of patient health to support triage and care planning. Through a referral code workflow, patients are securely linked to their care team, enabling virtual nurses to review data, generate clinical summaries, and support timely triage and follow-up with patients or their clinicians.
